For Your First Visit
Please bring completed new patient form or arrive 20 minutes early to fill out.
Allow 1.5-2 hours for your initial consultation and treatment. 

 
For Your First Visit and Follow-up Appointments
Allow 1-1.5 hours for follow-up treatments.                                                                                    Always eat something at least three hours prior to your appointment.
Wear loose clothing. We also provide gowns for your comfort.
Expect to become deeply relaxed and/or fall asleep during the treatment.
Avoid heavy physical exercise 2 hours after your acupuncture treatment.
Try to allow yourself some time to relax after your treatment. 

 
Payment Policy
Payment is expected at time of treatment. We accept VISA, MASTERCARD, DISCOVER, CHECKS and CASH as payment.

I am an in network provider for some PPO insurance providers. See list under Clinic Information page.

Cancellation
We ask you to provide us 24 hours notice of appointment changes or cancellation.  A fee will be charged for no shows and last minute cancellations. This is a courtesy to the provider as well as other patients who may be able to schedule in your absence.
